Selective Dorsal Neurectomy (SDN) for Premature Ejaculation

Premature ejaculation is one of the most frequently encountered problems in male sexual health. Although it is generally attributed to psychological factors, resistant and lifelong cases are usually based on an anatomical problem: excessive sensitivity of the glans penis, known as penile hyperaesthesia. The modern operation performed to balance this structural sensitivity by microsurgical means and to lengthen ejaculation time permanently is called Selective Dorsal Neurectomy (SDN).

What Is SDN (Selective Dorsal Neurectomy) Surgery?

Selective Dorsal Neurectomy is the reorganisation of the superficial nerve network, the dorsal nerve branches, that transmits the sensation of touch in the penis to the brain.

In men experiencing excessive sensitivity, the nerve network in the region of the glans is far denser than normal. This excess causes the friction of intercourse to reach the brain as an overwhelming stimulus, and the ejaculation reflex is triggered before it can be controlled. The underlying philosophy of the SDN operation is not to numb but to normalise the stimulation threshold. In a procedure performed under a surgical microscope, only the excess nerve branches responsible for the hypersensitivity are eliminated. In this way the patient gains substantial control over ejaculation time without any loss in the quality of orgasm.

Advantages Over Conventional Methods

Compared with products that provide only temporary delay, Selective Dorsal Neurectomy is a medical solution that addresses the source of the problem directly:

  • Continuous and lasting effect: because the treated nerve branches do not rejoin, the control and the increase in duration obtained after the operation continue for life.
  • An end to dependence on medication and sprays: it removes the obligation created by local anaesthetic creams or antidepressant-derived tablets taken before every sexual encounter, and restores a sense of naturalness to sexual life.
  • Partner comfort: delaying products often cause numbness or discomfort for the partner as well. Because SDN surgery leaves no need for any external chemical, it also maximises partner comfort.

Who Is Selective Dorsal Neurectomy Suitable For?

The greatest factor in the success of this treatment is correct patient selection. At our clinic the decision to operate is taken after a detailed urological assessment. The ideal candidates for this operation are:

  • Men who have experienced premature ejaculation from their first sexual experience onwards, that is lifelong.
  • Patients in whom excessive sensitivity of the glans penis is identified during andrological examination.
  • Those who respond well to delaying sprays, creams or condoms but do not wish to use these products for life.
  • People who do not benefit from medical (tablet) treatments or cannot tolerate their side effects.

The Operation and the Recovery Period

SDN operations carried out by Op. Dr. Taylan Yanar are planned according to advanced microsurgical principles with patient comfort as a priority.

1. The microsurgical stage

So that the patient experiences no stress, the procedure is mostly performed under regional anaesthesia (local anaesthesia) or with light sedation. A millimetric incision, which is not noticeable from the outside once healed, is made on the underside of the glans. Thanks to the operating microscope the sensory nerve branches are greatly magnified and only the fine branches causing excessive stimulation are deactivated. The process is generally completed in around 45 to 60 minutes.

2. Effect on erection and reproductive health

The deep tissues that pump blood into the penis and provide erection (the cavernous structure) and the subcutaneous sensory nerves on which the SDN procedure is performed are entirely different anatomical regions. For this reason the operation has absolutely no harm or effect on erectile capacity, sexual desire or sperm quality.

3. Return to daily life

SDN is a day-case procedure that does not require an overnight hospital stay. After resting for a few hours following surgery, patients can walk to their home or hotel. Return to everyday tasks and desk work is possible within one to two days. Approximately four weeks of sexual abstinence is essential so that the wound heals properly and the brain adapts to the new level of sensitivity.
