What Are Shock Wave (ESWT) and SVF Treatment?
Erectile dysfunction and Peyronie's disease arise from narrowing of the vascular structure of the penis, loss of elasticity, or the tissues becoming fibrous and forming plaque. Conventional drug treatments provide a temporary increase in blood flow; shock wave therapy (ESWT) and SVF (stromal vascular fraction) stem cell treatment aim instead to repair the tissue at a biological level.
Both of these cellular and tissue regeneration methods are applied by Op. Dr. Taylan Yanar at the Mavisehir Andrology Clinic in Izmir, and are explained in full below.
What Is Shock Wave Therapy (ESWT)?
Shock wave therapy (ESWT — extracorporeal shockwave therapy) is the application of low-intensity focused sound waves to the tissue of the penis using a special device. It creates no incision, no burning and no damage to the body.
How it works: the low-intensity sound waves stimulate blocked or damaged capillaries in the penis. This stimulus triggers the body's natural healing mechanism and leads to the formation of new capillaries in the area (angiogenesis).
How it is applied: the treatment is carried out in the clinic and requires no anaesthetic. Each session lasts approximately 15 to 20 minutes. It is generally planned as a course of 6 to 12 sessions, at two or three sessions per week.
Where it is used: it is highly effective in mild to moderate erectile dysfunction, in patients with diabetes or high blood pressure, and in softening plaque during the early stage of Peyronie's disease.
What Is SVF (Stem Cell) Treatment?
SVF treatment (stromal vascular fraction) is the injection into the penile tissue of a live cell suspension, rich in stem cells and growth factors, obtained from the patient's own abdominal fat.
How it works: the millions of stem cells contained in the SVF fluid identify the damaged inner lining of the blood vessels (the endothelium) and the nerve endings within the penis, and begin the process of biological repair. This increases the capacity of the penis to retain blood.
How it is applied: a small quantity of fat tissue is taken from the patient's abdomen under local anaesthetic. After being processed through specialised laboratory centrifuge and separation stages, the resulting stem cell fluid is injected into the channels of the penis. The procedure is completed within the same day.
Where it is used: it is applied in advanced vascular loss of erectile function, in patients whose tissue elasticity has diminished, and in the calcified plaques of Peyronie's disease.
How Are ESWT and SVF Treatment Applied Together?
Shock wave therapy creates micro-stimuli in the penis and so increases blood supply to the tissue, while SVF stem cell treatment delivers fresh repair cells to that stimulated area. In clinical cases, combining the two methods achieves maximum and long-lasting biological improvement in the quality of erection.
Frequently Asked Questions
Are shock wave and SVF treatments painful?
ESWT is an entirely painless procedure. In SVF treatment, local anaesthetic is applied during both the fat harvesting and the injection stages, so the patient feels no pain.
When do these treatments start to work?
With ESWT, the effects are noticed from the third or fourth session onwards. With SVF stem cell treatment, tissue regeneration begins from the first month and reaches its maximum within three to six months.
