Is Erectile Dysfunction Caused by Blood Vessels?

When erectile dysfunction has an organic cause, it stems largely from vascular factors. Erection is entirely a circulatory mechanism: with sexual arousal the arteries dilate and pump blood at high pressure into the spongy tissue, and that blood is then trapped by the veins.

A fault at any stage of this vascular cycle causes the erection either never to form or to be lost shortly after it begins.

The Two Basic Vascular Problems That Impair Erection

Vascular erectile dysfunction is examined in two main groups:

  • Arterial narrowing (arterial insufficiency): narrowing of the arteries that carry blood to the penis. Hypertension, high cholesterol, smoking and diabetes cause plaque to form on the inner wall of the vessels (atherosclerosis). Because blood cannot reach the penis in sufficient quantity or at sufficient speed, an erection cannot be initiated.
  • Venous leak (venous insufficiency): blood entering the penis must remain locked within the spongy tissue (corpus cavernosum) throughout intercourse. If the venous valves lose their function, blood cannot be retained and escapes back into the systemic circulation. This causes the erection to subside within seconds even after it has begun.

Why Is Vascular Erectile Dysfunction a Warning Sign for Heart Attack?

The arteries of the penis are on average 1 to 2 millimetres in diameter, whereas the coronary vessels supplying the heart are 3 to 4 millimetres. Although atherosclerosis begins throughout the body at the same time, it gives its first sign in the penile vessels, which have the smallest diameter.

Medical research shows that men with vascular erectile dysfunction may face the risk of coronary heart disease or a cardiac event within three to five years if no precautions are taken. For this reason erectile dysfunction is not only a sexual problem; it is an early warning system for general vascular health.

How Is Vascular Erectile Dysfunction Diagnosed and Treated?

At our Andrology Clinic in Mavisehir, Izmir, vascular damage is identified and treated through the following steps:

1. Diagnosis: Colour Penile Doppler Ultrasound

To establish definitively whether erectile dysfunction is caused by arterial narrowing or by venous leak, colour penile Doppler ultrasound is performed. After a special injection into the penis, blood flow velocities and resistance values in the vessels are measured to the millimetre.

2. Modern Treatment Methods

Depending on the degree of damage, individually planned regenerative and therapeutic protocols are applied:

  • ESWT (shock wave therapy): low-intensity sound waves applied to the penis open blocked capillaries at micro level and trigger the formation of new vessels (angiogenesis).
  • Stem cell (SVF) and exosome treatments: repair the damaged inner vessel wall (endothelium) and elastic tissues at cellular level.
  • P-Shot (PRP): improves tissue nutrition by revitalising circulation.
  • Three-piece penile prosthesis: a 100 per cent permanent solution for patients with advanced vascular damage who do not respond to non-surgical methods.

Frequently Asked Questions

How does diabetes lead to vascular erectile dysfunction?

High blood sugar damages the endothelial tissue lining the inner surface of the vessels and impairs the release of nitric oxide, which dilates them. This prepares the ground for both arterial narrowing and venous leak.

How can you tell whether erectile dysfunction is vascular in origin?

The clearest sign is the complete disappearance of night-time and early-morning erections.

If the involuntary erections that occur at night or on waking have disappeared completely, and the person also has hypertension, a smoking habit or high cholesterol, the probability that the problem is vascular is very high. A definitive diagnosis is made with penile Doppler ultrasound.
